Priorities in this case
Rather than creating definition by reducing superficial facial fat, the priority was to preserve the patient’s natural volume while correcting the position of the descended deeper tissues.
Lower-face heaviness, jowls, nasolabial folds and the transition from beneath the chin into the neck were not treated as separate concerns. The cheek and neck were evaluated as one continuous unit, with the aim of improving contour definition without changing the patient’s characteristic facial appearance.
Procedure selected and its purpose
Deeper Plane Face & Neck Lift
This operation was designed to reposition and reconstruct descended tissues at a structural level deeper than that addressed in a conventional Deep Plane procedure. The aim was to correct structural descent from the cheek to the neck without placing excessive tension on the skin surface.
Whether facial liposuction should be combined with the lift is determined according to the amount and distribution of fat, skin condition and skeletal anatomy. In this case, the strategy was to avoid excessive reduction of superficial volume and instead correct the position of the deeper tissues.
Changes visible at three months
In the oblique view, heaviness around the jowl is less prominent than before surgery, and the transition from the cheek to the jawline is more readily defined. In the profile view, the contour from beneath the chin into the neck is clearer. In the frontal view, the contour is improved while the patient’s characteristic facial appearance is preserved.
At three months, swelling is beginning to settle, although scars, tissue firmness and sensation may continue to change. Results and recovery vary between patients, and the same degree of change cannot be guaranteed.

Principal risks, adverse effects and complications
Possible risks include bleeding, hematoma, infection, visible scarring, delayed wound healing, hair loss around the incision, prolonged swelling, bruising or edema, pain, tightness, altered sensation, nerve injury including facial nerve dysfunction, contour irregularity, asymmetry, recurrence of laxity and a result that differs from the patient’s expectations.
